Tadalafil can be taken with or without food because an ordinary meal does not have the clinically important delaying effect associated with some shorter-acting PDE5 inhibitors. Older age alone does not usually require a tadalafil dose adjustment, but kidney or liver impairment can reduce clearance and make a 20 mg dose or frequent use unsuitable. Elimination and Half-Life

The medicine is metabolised mainly in the liver through the CYP3A4 enzyme pathway, producing metabolites that do not make a meaningful contribution to its clinical effect.

Tadalafil has a mean elimination half-life of approximately 17.5 hours in healthy adults. This is considerably longer than the half-lives of sildenafil and vardenafil and helps explain why responsiveness can persist for up to 36 hours. Elimination may be slower in some older adults and in people with kidney or liver impairment. Most of a dose is ultimately excreted as metabolites in the faeces, with a smaller proportion excreted in urine.

Tadalafil can remain in the body long after the immediate dosing period, so its interactions remain relevant during the following day and, in some cases, longer. In an emergency, tell medical staff when the last tadalafil dose was taken, particularly if chest pain treatment is needed.

Side Effects and When to Seek Help

Many people take tadalafil without troublesome adverse effects. When effects occur, they are often mild to moderate and resolve as the medicine leaves the body. Headache, indigestion, flushing, nasal congestion, back pain, and muscle aches are among the most frequently reported. Back or muscle discomfort may appear several hours after dosing and generally settles without lasting harm.

  • Common effects include headache, facial flushing, indigestion, nasal congestion, back pain, muscle aches, and mild dizziness.
  • Less common effects include blurred vision or other visual disturbance, ringing in the ears, palpitations, a rapid heartbeat, nosebleed, reflux, nausea, and changes in blood pressure.
  • Rare but serious reactions include severe allergic reaction, sudden vision loss, sudden reduction or loss of hearing, serious cardiovascular events, and priapism.

Stop sexual activity and seek urgent medical care if chest pain, marked breathlessness, fainting, or severe dizziness occurs. Do not take a nitrate for chest pain after tadalafil. Sudden loss of vision in one or both eyes or sudden hearing loss, sometimes accompanied by ringing in the ears or dizziness, also requires immediate assessment. An erection that is painful or lasts more than four hours is a medical emergency because delayed treatment can permanently damage erectile tissue.

Overdose and Accidental Exposure

Clinical studies have administered single tadalafil doses well above the usual 20 mg dose to healthy volunteers. Adverse effects were generally similar in type to those seen at therapeutic doses but could occur more often or more intensely. Possible features include severe headache, flushing, indigestion, dizziness, low blood pressure, fainting, visual symptoms, and a prolonged erection. There is no specific antidote.

Treatment is supportive and depends on symptoms, blood pressure, cardiovascular status, and the amount and timing of exposure. Because tadalafil is highly bound to plasma proteins, haemodialysis does not meaningfully accelerate removal. Anyone who has taken more than intended, or any child who may have swallowed a tablet, should obtain prompt advice even if symptoms have not yet appeared. In Australia, call the Poisons Information Centre on 13 11 26; call emergency services immediately for collapse, severe chest pain, breathing difficulty, loss of consciousness, or a prolonged erection.

Storage

Store Tadalis SX at 25°C; brief excursions from 15°C to 30°C are permitted. Keep the tablets in their original container, protected from moisture, and out of the sight and reach of children. Avoid bathrooms, vehicles, and other places exposed to heat or humidity. Do not use a tablet after the expiry date printed on the pack or if its packaging is damaged. Unwanted or expired medicine can be returned to a pharmacy for safe disposal rather than placed in household rubbish or flushed into wastewater.

Benefits and Trade-offs

The principal practical advantage of tadalafil 20 mg is duration. A single dose can support erectile responses for up to 36 hours, allowing more flexibility than a medicine that must be closely timed to one planned encounter. Food has little clinically meaningful effect on tadalafil absorption, which can also make use around meals more straightforward. These characteristics may reduce time pressure and suit couples who prefer a broader opportunity for sexual activity.

The same long half-life creates trade-offs. Headache, back pain, indigestion, blood-pressure effects, and medicine interactions can remain relevant for longer than with a short-acting option. The extended window also means that nitrates remain unsafe for at least 48 hours after a dose. A 20 mg tablet is not the best starting dose for every patient, cannot reproduce the flexibility of commercially available lower strengths, and should not be taken repeatedly because an immediate response has not occurred.

Response varies, and no PDE5 inhibitor works for everyone.
